What it is

Can hyaluronic acid be removed from the lips?

Yes, and it is one of the great advantages of this material compared to any other filler. Hyaluronic acid can be dissolved in a controlled manner with an enzyme called hyaluronidase, which breaks down its chains and allows the body to eliminate it. The lip is not damaged or scarred: it returns to the shape it had before the first injection.

Furthermore, the lips are the area where this treatment is most frequently requested. It is a small, highly visible area in constant motion, with an anatomy that does not forgive excess product or a poorly chosen technique. Consult Alldent about hyaluronic acid removal treatment as a complement to your dental treatment.

What hyaluronidase is and how it works

It is the same enzyme that your body already uses to naturally reabsorb the filler over the months. It is injected into the exact plane where the product is located, acting upon it and degrading it into fragments that the body eliminates without leaving any residue.

It only works with hyaluronic acid fillers

Hyaluronidase does not act on permanent or semi-permanent materials: silicone, methacrylate, calcium hydroxyapatite, or polycaprolactone do not dissolve with this enzyme and require a completely different approach. That is why the first thing we do in the consultation is find out what was injected. If you do not have the report, the examination and clinical history guide us, but knowing for certain changes the treatment.

When it is dissolved

When it is advisable to remove hyaluronic acid from the lips

Not every result that is disliked needs to be dissolved. Sometimes it is enough to wait, and sometimes it is better corrected by adding product at another point rather than removing it. These are the cases where removal is indeed the answer.

01

Excess volume or duck lips

The most frequent reason for consultation. Too much product, or product well-placed but in a quantity that does not fit with the rest of the face. It is not necessary to remove it all: in many cases, we inject a tailored dose to reduce without eliminating, maintaining a moderate result that you do like.

02

Product migrated above the lip border

The filler has moved away from where it was injected and has risen above the vermilion border, creating that blurred ‘shelf’ around the mouth. Migration is not corrected by adding more product: the displaced filler must be removed and, if you wish, re-injected weeks later using the correct technique.

03

Lumps, nodules, and encapsulated hyaluronic acid

Palpable lumps in the lip, sometimes visible against the light or when pressed, which appear when the product clumps together or the body isolates it by forming a capsule. Hyaluronidase is the treatment of choice in most of these cases, although a nodule with redness, heat, or pain is assessed before injecting anything.

04

Asymmetrical lips

When one side clearly has more product than the other and the difference cannot be resolved by compensating. It is dissolved selectively on the side with the excess, using a very small dose and constant visual monitoring during the session.

05

Vascular complication (emergency)

If the product compresses or occludes a vessel, hyaluronidase is applied urgently and in high doses. Warning signs include intense, disproportionate pain, paleness or purple spots in the area, and vision disturbances. If you have just had an injection and notice any of these symptoms, do not wait: go to an emergency department immediately.

/ 01

Medical assessment and diagnosis

Lip examination, palpation to locate accumulations and nodules, and a review of your history: what product was injected, when, how many sessions, and with which technique. We also review allergy history and medication. From this comes the decision to dissolve or not, the estimated dose, and the predictable number of sessions.

/ 02

Informed consent and plan

Before injecting anything, we explain in writing what will happen, what result is realistic, and what risks exist. This includes something that is worth understanding beforehand: when dissolving, the lip does not return to how it was last week; it returns to how it was before the very first filler. If you have been having injections for years, that can be surprising.

/ 03

Hyaluronidase injection

A 30-minute session with topical anaesthesia. It is injected into the plane where the product is located, at several points and with a dose adjusted to the objective: total removal or partial reduction. We work with conservative doses because the enzyme also temporarily affects the tissue’s own hyaluronic acid, and overdoing it means losing more volume than intended.

/ 04

The first 48 hours

It is common for the lip to swell significantly immediately afterwards, more than you might expect, and for the appearance to be worse than before you arrived for one or two days. This is a normal reaction to the enzyme. The swelling goes down between the second and third day, and that is when the real effect begins to be seen.

/ 05

Two-week review

We schedule an appointment to see the settled result. If residual product remains, it is completed in a second session: this is frequent and planned for, especially with old, highly cross-linked, or encapsulated product.

/ 06

Re-injecting, if you wish

If your intention is to be treated again but correctly, we wait between two and four weeks from the last dose of enzyme. Before that period, part of the new product would be degraded by the hyaluronidase that still remains active in the tissue.

What our patients think

Frequently asked questions about removing hyaluronic acid from the lips

By injecting hyaluronidase, an enzyme that breaks down the filler chains so the body can eliminate it. It is applied in the clinic with topical anaesthesia, in a 20 to 30-minute session, in the same plane where the product is located. There is no surgery, no incisions, and no other way to remove it: there is no cream, massage, heat, or oral treatment that dissolves a filler once injected.

The effect begins within the first few hours, but it is not visible until the swelling goes down. In practice: on days 1 and 2, the lip is swollen and nothing can be assessed; from the third day, the reduction can be seen, and the final result is achieved between the second and third week. With old or highly cross-linked product, more sessions may be needed.

It is a safe treatment when performed by a professional in a healthcare environment, but it is not harmless. The relevant risk is an allergic reaction, which is rare but potentially serious, and that is why it is applied in a clinic with emergency medication available and a prior assessment of history. The real risk is not in the enzyme: it is in applying it without medical judgement, without knowing what product is being dissolved, or in a place without the capacity to respond to a complication.

It depends on the amount of product to be dissolved and whether one or two sessions are anticipated. We provide a fixed price during the medical assessment, which is free and without obligation. If during that visit we conclude that your case does not require product removal, we will tell you and you will have paid nothing.
